List of corporate and foundation contributors
and amounts contributed;

Identify if organization is a CDFI or a CDC;

Governance information; |
 |
List of officers and Board of Directors; |

Marketing/Communications materials such as Annual Report
(if available) or brochures describing programs and services; |

Itemized project costs related to the program;
 |

Information about the method in which the contribution
will be used; and |
 |
Audited financial statements. (minimum 2 years) |

Letters of Intent (LOIs) are accepted on a quarterly basis. Deadlines are March 1 and August 1. LOIs should include the following information:

Proposals are accepted on a quarterly basis.Application deadlines are March 1 and August
1. Please provide proposals in tri-plicate or 3 copies.
Incomplete packages may cause considerable delay of funding and in some instances suspension. Thank you.
Requests for capital campaigns will be received by the Foundation and directed to a
department of Cathay Bank for consideration.
Organizations are limited to grant funding to once in a calendar year. |
